Endometriosis is a chronic gynaecological condition that affects many women in Singapore. It occurs when tissue similar to the lining of the uterus grows outside the womb, causing pain, infertility, and other symptoms. Understandably, many women hope for a quick and easy way to confirm whether they have endometriosis. One of the most common questions asked is whether an endometriosis blood test exists.

Is There a Reliable Endometriosis Blood Test?
At present, there is no single reliable blood test that can accurately diagnose endometriosis. While researchers have been studying various blood markers — such as CA-125 — these tests are not specific or sensitive enough to be used as standalone diagnostic tools. CA-125 levels may be elevated in women with endometriosis, but they can also rise due to other conditions, such as ovarian cysts, fibroids, or even menstruation.
Therefore, a blood test alone cannot confirm whether a woman has endometriosis.
How Is Endometriosis Diagnosed?
Although an endometriosis blood test would be ideal, diagnosis currently relies on a combination of methods:
1. Symptom Evaluation
Doctors will begin by asking about common symptoms such as:
- Pelvic pain, especially during menstruation.
- Pain during or after sexual intercourse.
- Infertility.
- Painful bowel movements or urination.
- Chronic fatigue.
The nature and severity of symptoms can vary greatly from one woman to another, which is why further investigations are often necessary.
2. Pelvic Examination
A gynaecologist may perform a pelvic exam to feel for abnormalities, such as cysts or scarring behind the uterus. However, smaller areas of endometriosis may not be detectable this way.
3. Imaging Tests
Ultrasound scans, especially transvaginal ultrasounds, can help detect ovarian endometriomas (a type of cyst linked to endometriosis). While ultrasounds are useful for identifying larger or deeper lesions, they may miss smaller or superficial patches of endometriosis.
In more complex cases, magnetic resonance imaging (MRI) may be used for a more detailed view.

4. Laparoscopy (Keyhole Surgery)
Laparoscopy is the gold standard for diagnosing endometriosis. It is a minimally invasive surgical procedure in which a camera is inserted into the abdomen to look for endometriotic tissue. If needed, a biopsy can be taken during the same procedure.
Although laparoscopy offers the most accurate diagnosis, it comes with surgical risks and is usually considered after non-invasive tests and symptom management have been explored.
Why Early Diagnosis Matters
Delays in diagnosis are common — many women suffer for years before receiving a formal diagnosis. Early diagnosis and management can help reduce pain, preserve fertility, and improve quality of life. If you are experiencing symptoms, it is important to speak to a gynaecologist experienced in treating endometriosis.
