What Is A Stroke?

A stroke occurs when blood flow to a part of the brain is interrupted, depriving brain cells of oxygen. This can lead to serious brain damage, long-term disability, or death. There are two main types of stroke:

  • Ischaemic stroke (IS): Caused by a blockage in an artery supplying blood to the brain.
  • Haemorrhagic stroke (HS): Caused by the rupture of a blood vessel, leading to bleeding in the brain.

Early intervention is vital—strokes are medical emergencies, and time is brain.

Singapore Stroke Statistics

According to the Singapore Stroke Registry 2022 Annual Report, strokes are on the rise:

  • Stroke episodes increased by over 50%, from 6,367 in 2012 to 9,702 in 2022.
  • The median age at stroke onset rose to 70.2 years, up from 67.6 in 2012.
  • Ischaemic strokes made up approximately 80% of all strokes; the remaining 20% were haemorrhagic strokes.
  • In 2022, more males (58%) experienced strokes than females (42%), and Malays had the highest age-standardised incidence rate (ASIR) at 240.2 per 100,000 population.

Causes Of Strokes

The major causes and risk factors of strokes in Singapore include:

  • Hypertension
  • Hyperlipidaemia
  • Diabetes mellitus
  • Smoking
  • Atrial fibrillation

These are often linked to lifestyle and can be prevented or managed with early intervention.

Symptoms Of Stroke

Use the F.A.S.T. acronym to recognise the signs of stroke:

  • Face: Drooping on one side of the face
  • Arms: Weakness or numbness in one arm
  • Speech: Slurred or confused speech
  • Time: Call 995 immediately

Other symptoms may include sudden vision loss, dizziness, or difficulty walking.

Screening & Prevention

Preventive measures play a key role in reducing the risk of strokes:

  • Regular blood pressure and cholesterol checks
  • Healthy diet low in salt and saturated fats
  • Regular physical activity
  • Smoking cessation programmes
  • Management of chronic conditions like diabetes and heart disease

Annual health screenings are encouraged, especially for those over 40 or with a family history of stroke.

Stroke Treatments

Treatment depends on the type and severity of the stroke:

1. Ischaemic Stroke

  • Thrombolytic therapy (clot-busting drugs) must be administered within 4.5 hours.
  • Endovascular thrombectomy (EVT) is available for certain patients.
  • Anti-platelets and anti-coagulants are commonly prescribed.

2. Haemorrhagic Stroke

  • Managed through surgery, endovascular treatment, or critical care monitoring.

In 2022, 5.8% of IS patients received thrombolytic agents, and 5.3% underwent EVT.

Costs Of Stroke Treatment

In Singapore, stroke care costs can vary widely:

  • Initial hospitalisation can cost from S$3,000 to S$10,000 or more, depending on ward class, hospital and treatment type.
  • Clot-busting Drugs (Thrombolysis) can cost from S$5,000 to S$10,000
  • Mechanical Thrombectomy may cost from S$20,000 to S$30,000
  • Surgical Treatments can costs from S$30,000 to S$50,000
  • Rehabilitation (physiotherapy, occupational therapy, speech therapy) may cost from S$3,000 to S$10,000 per month
  • Medications and follow-up consultations add to ongoing expenses.

MediSave, MediShield Life, and Integrated Shield Plans can help offset costs. Subsidies are available in public hospitals for eligible patients.

MediSave For The Chronic Disease Management Programme (CDMP)

MOH explains that “The Chronic Disease Management Programme (CDMP) was first introduced in 2006 to help patients with chronic conditions to reduce out-of-pocket expenses for outpatient management of their conditions and help to promote health-seeking behaviours. Eligible patients can utilise their MediSave at MediSave-accredited healthcare entities (all polyclinics, all public hospitals’ specialist outpatient clinics, accredited specialist clinics, and accredited GP clinics) and subsidies at CHAS GP clinics for their chronic conditions.

These conditions often lead to more serious complications if detected late or not followed up regularly.  Individuals are therefore encouraged to have a regular family doctor, undergo the recommended health screenings and have regular reviews of their chronic condition, timely medical treatment and lifestyle changes.”

Check against MOH / CPF or oyur healthcare provider for the latest policy on MediSave usage for stroke treatment.
