Colectomy – What You Need To Know About Colon Cancer Surgery

Colectomy

A colectomy is a surgical procedure to remove all or part of the colon, also known as the large intestine. It is one of the most common and effective treatments for colon cancer, particularly when the disease is detected in its earlier stages.

Types Of Colectomy

There are different types of colectomy, depending on how much of the colon needs to be removed:

  • Partial Colectomy (Segmental Resection): Only the cancerous section and a margin of healthy tissue are removed.
  • Total Colectomy: The entire colon is removed.
  • Hemicolectomy: Either the right or left portion of the colon is removed.
  • Proctocolectomy: Both the colon and rectum are removed (usually for rectal cancer or inflammatory bowel disease).

The goal of colectomy in colon cancer cases is to eliminate the tumour, reduce the risk of spread, and examine nearby lymph nodes for staging and prognosis.

When Is A Colectomy Needed?

Doctors typically recommend a colectomy when:

  • Cancer Is Localised: The tumour is confined to one part of the colon and can be safely removed.
  • Precancerous Growths Exist: Large or high-risk polyps that cannot be removed during a colonoscopy may warrant surgery.
  • Bowel Obstruction Or Perforation: Some patients develop blockages or ruptures in the colon due to a tumour, requiring emergency colectomy.
  • Recurrent Bleeding Or Anaemia: Chronic blood loss from a tumour site may necessitate surgical removal.
  • High Genetic Risk: Patients with hereditary conditions like familial adenomatous polyposis (FAP) or Lynch syndrome may undergo preventative colectomy.

What Happens During The Surgery?

Colectomy can be performed through open surgery (a single large incision) or laparoscopic surgery (keyhole technique using small incisions and a camera). The choice depends on the tumour size, location, and the patient’s overall health.

During the procedure:

  • The surgeon removes the affected portion of the colon.
  • Nearby lymph nodes are taken out and tested for cancer spread.
  • The remaining ends of the colon are either reconnected (anastomosis) or brought out as a stoma on the abdomen (colostomy), depending on the case.

What To Expect After Colectomy

Hospital Recovery

  • Patients typically stay in hospital for 3 to 7 days, depending on the type of surgery and recovery progress.
  • Pain management, early walking, and a gradual return to solid food are part of the recovery plan.

Home Recovery

  • Full recovery may take 4 to 6 weeks.
  • Bowel habits may temporarily change, including more frequent or looser stools.
  • A stoma (if created) requires care and lifestyle adjustments, often supported by stoma nurses and education.

Follow-Up

  • Your care team will discuss pathology results and whether further treatment, such as chemotherapy, is needed.
  • Regular surveillance with colonoscopy and scans will be arranged to monitor for recurrence.
