Limitations Of CA19-9 As A Screening Tool

Lack Of Specificity (False Positives)
A significant drawback of CA19-9 is that it is not exclusive to pancreatic cancer. Elevated levels are frequently found in several benign (non-cancerous) conditions, which can lead to unnecessary anxiety and invasive follow-up tests.
- Biliary Obstruction: Gallstones, jaundice, or bile duct inflammation (cholangitis) can cause CA19-9 levels to spike significantly.

Screening tools are most effective when they can catch “Stage 1” disease. However, CA19-9 is often only significantly elevated in the advanced stages of pancreatic cancer. By the time the marker is high enough to be noticed in a routine screen, the cancer may have already progressed to a point where surgical resection is more difficult.
Poor Positive Predictive Value (PPV)
Because pancreatic cancer is relatively rare in the general population compared to benign conditions, the Positive Predictive Value of a CA19-9 test is extremely low (often estimated at less than 1%). This means that if an asymptomatic person has an elevated result, there is a less than 1% chance that it is actually caused by pancreatic cancer.
